PRIME Performance Health is a health advisory service operating in Australia. This policy explains how we collect, use, store, and protect your personal information in accordance with the Australian Privacy Principles under the Privacy Act 1988 (Cth).

What We Collect

We collect information you provide during the application and intake process. This includes your contact details and health goals submitted at application, and your clinical intake responses covering current health status, medical history, lifestyle, and priorities. We collect only what is necessary to assess whether and how we can help you.

How We Use Your Information

Your information is used solely to evaluate your application, conduct your clinical intake assessment, and coordinate care recommendations with practitioners you identify. We do not use your information for marketing, profiling, or any purpose unrelated to your direct care.

How Your Data Is Transmitted and Stored

All information submitted through PRIME is transmitted over TLS (HTTPS) — encrypted end-to-end between your browser and our servers. Your data is stored in a secure database hosted in an Asia-Pacific data centre with access restricted to authorised PRIME personnel only. Clinical intake data is stored separately from any analytics or marketing systems. For information about the specific hosting location, contact us directly.

Secure Intake Access

Access to your intake forms is provided via a secure, time-limited link sent directly to you. This link expires after 90 days and is automatically deactivated once your intake is complete, ensuring only you can access and submit your information.

Analytics

Google Analytics is used on the public landing page only, to measure aggregate visitor traffic. It is not present on any intake or clinical pages. No health information or personal intake data is ever transmitted to Google Analytics.

Data Sharing

Your information is never sold, rented, or shared with third parties for commercial purposes. Data may be shared only where necessary to deliver the service — for example, with referring practitioners you have identified — or where required by Australian law.

Data Retention and Deletion

We retain your information for as long as necessary to provide the service and meet our legal obligations. If your application is not accepted, or if you request deletion of your information, your personal data will be removed from our systems within 30 days of that request. Some information may be retained in de-identified form for clinical quality purposes.

Under the Australian Privacy Principles, you have the right to request access to, correction of, or deletion of your personal information held by PRIME. To exercise any of these rights, contact us using the details below. We will respond within 30 days.
